A hook connects from the side and swivels the head. That rotation is what causes the damage: the skull stops fast, the brain lags behind, and tissue shears. Angular acceleration, not raw force, is the bigger driver of concussion.
Conventional headgear is foam bonded straight to the shell, so the padding and the skull move together. That helps very little against a glancing or rotational shot.
Football and cycling helmets added a layer for this years ago. Boxing headgear hasn't caught up.
